Care Navigators to help people keep on track with their health

The Far West Local Health District and general practices in Broken Hill are working together to offer a program to help people at risk of developing long-term disease to make changes to their lifestyles to stay heathy.

The Staying Healthy Program is an early intervention program designed to help people at risk through lifestyle choices and it now has ‘Care Navigators’ at two local GPs to help people have easier access to and navigate the services they need and to follow up with their GP on a regular basis.

“The Staying Healthy Program puts you at the centre of your care, taking into account what matters to you, and the Care Navigators will help people connect with services and support you to share information to improve your health,” said Denise McCallum, Project Manager Integrated Care Far West LHD.

Two general practices – Nachiappans and Broken Hill GP Super Clinic – now have Care Navigators available to help people. The new Care Navigators are Liz Cross at Nachiappans Surgery and Molley Jones at The Broken Hill GP Super Clinic.

“The Care Navigators will help by making it easier to access and navigate the people and services you need and to follow up with your GP on a regular basis,” said Ms McCallum. “The program will also assist you through and access to information, services and providers you need, and health coaching.”

The Staying Healthy Program will target 20-55 year olds living in Broken Hill to support them to access and navigate the people and services they need to make changes to their lifestyles.

Some of the lifestyles that can put people at risk of a developing a long-term disease include:

  • Smoking
  • Not being active
  • Poor diet choices
  • Being overweight
  • Drinking too much alcohol

“Changes to these lifestyles can reduce your risk of developing long-term diseases such as diabetes, heart disease and improve your overall health and wellbeing,” said Ms McCallum.
